Summer Enrollments
Cambridge High School will accepting enrollment packets for students looking to enroll summer for fall 2016. Individual appointments with counselors will be available starting July 19th. Please call the Front Office at 470-254-2883 to book an appointment. The enrollment packet is available to be picked up in the Counseling Office from now until May 27th and in the Front Office all summer.

ALL REQUIRED DOCUMENTS ARE NEEDED TO ENROLL.
To expedite the enrollment appointment please see the points below:
- Bring all documents required to enroll (see below). ALL documents are required to complete enrollment.. AND
- Complete the enrollment packet in advance. Enrollment packets are available for pick-up in the office.. AND

DOCUMENTS REQUIRED TO ENROLL-
Academic Records: Withdrawal Form from previous school and transcripts or final grade reports are required. We cannot schedule classes without these documents. Students in grades 7-12 will also need copies of any discipline records or a letter from previous school stating that there is no discipline on file and Attendance Record.
Math Documentation (not required for enrollment but needed for proper placement). Please bring a full course description of all math courses attempted in high school (and middle school if rising 9th grader) with standards and topics listed for correct math placement
- Transcript/Final Grade Report (for students starting 9th grade, the transcript or grade report needs to state that the student is PROMOTED to 9th grade) We cannot schedule classes without a full grade history from previous school.
- Attendance Record
- Discipline Record or letter from school stating that there is no discipline on file.
Ear, Eye and Dental Screening: Students entering a Georgia school for the first time must have a certificate of eye, ear, nutrition, and dental screening. This Georgia Department of Human Resources certificate (Form 3300) must be signed by the Health Department or a licensed Georgia physician.
Georgia Immunization Certificate (Form 3231): Georgia requires students to be immunized against measles, mumps, polio, rubella, whooping cough, tetanus, diphtheria, hepatitis B, haemophilus influenza, and varicella. Immunization information must be on the proper Georgia Department of Human Resources Form 3231; these are available through the Fulton County Department of Health and Wellness or local physicians. (NO Waivers available for summer enrollments)

TWO Proofs of Residency: Per Board Policy JBC, “School Admissions,” two documents indicating residence in the attendance area must be provided for all students (2012-13 Cambridge Attendance Zone). Appropriate records for verification of residency must include the following:
One current Water, Electric or Gas bill AND one Residence document from the list below.
Both documents must contain parent or guardian’s name and current address.
- Copy of home mortgage Bill
- Current bank statement
- Valid Driver’s License/ State ID with current address
- Current Paycheck
- Copy of home sale contract
- Current HOA Bill
- Section Eight/HUD Housing Document
- Current Apartment/House Lease
- Homeowner’s/Renter’s insurance registration card
Social Security Number: Social security numbers are voluntary. However, they are required to be on file for the HOPE scholarship eligibility.
Legal Guardianship Papers: If not living with natural parent.
**Includes students who are 18 or older; emancipated; adult students. Enrollment of any child who meets the definition of homeless is determined by guidelines in Fulton County Board of Education Policy JBCA.
Persons Residing with Another Homeowner/Renter
If the student and parent/guardian reside with another person(s) the homeowner/renter must be present and provide the two proofs as detailed above. After 30 days, two additional documents bearing the name and address of the parent/guardian must be provided.
